چکیده

SUMMARY Metabolic turnover studies performed on six patients with surgically induced biliary fistula given tritiated digoxin reveal virtually all stool excretion of digoxin to be derived from bile. This establishes the relative lack of recycling of digoxin as an important factor in digoxin metabolism. This is probably related to its polar chemical structure and is a determinant of its clinical duration of action in human subjects. The amount of digoxin in the enterohepatic circulation (recycling) was calculated to be 6.5% of the administered dose and absolute absorption to be 85% in human subjects. Additional Indexing Words: Biliary fistula Digoxin, metabolism of T HE availability of 3H digoxin for experimental clinical use has made possible a number of meaningful studies during the last decade.1-17 These studies have outlined the serum tumover rates, serum half-time (the time required for one half of the radioactivity identified with the drug to disappear from the serum) and serum digoxin levels. The urinary and fecal excretion of digoxin was characterized , and the larger portion of the drug was found to be excreted unchanged in the urine.

Studies with tritiated digoxin in anephric human subjects.

Tritiated digoxin was given to 11 patients when anephric prior to renal transplant. These patients exhibited high digoxin serum levels, increased digoxin serum half-times, and reduced excretion of digoxin compared to those of patients with congestive heart failure and normal BUN and are similar in this respect to patients with severe compromise of renal function. The stool excretion of digoxin ...

Tritiated digoxin excretion of patients following renal transplantation.

Four patients with a single allografted kidney were given 1.0 mg of tritiated digoxin intravenously. Compared with patients with two normally functioning kidneys, these patients cxhibited increased digoxin serum levels, modestly reduced renal excretion, and increased stool excretion of digoxin during a 7-day study. When administration of digoxin is indicated for a patient with a renal transplan...

The Excretion of Tritiated Digoxin

Six normal human volunteers as renal transplant donors were given 33H-digoxin before and again after unilateral nephrectomy. The serum digoxin concentration was modestly increased following nephrectomy, the serum digoxin half-time was slightly prolonged, and total digoxin excretion for the group was reduced by only 6.4% during a 7-day study. Four of the six patients studied before and after uni...

Tritiated Digoxin XVIII

Tritiated digoxin turnover studies were performed in nine children aged 4 days to 7 years. Mean serum turnover time was 32.5 (range 18-48) hours, and the 3-day urine and stool excretion was 55% of the total dose administered. These findings are quite similar to those observed in adults and indicate that the larger doses of digoxin required for digitalization in infants are not accompanied by in...

Interruption of the enterohepatic circulation of digitoxin by cholestyramine. II. Effect on metabolic disposition of tritium-labeled digitoxin and cardiac systolic intervals in man.

Previous studies of digitalis glycoside metabolism and excretion have indicated that these compounds undergo a significant enterohepatic cycle in some species. It has been suggested that the existence of such a cycle in man contributes to the prolonged action of certain cardiac glycosides. Previous studies have demonstrated that cholestyramine binds digitoxin and digoxin in vitro and accelerate...
